Treat and Prevent UTIs Without Drugs

Urinary tract infections (UTIs), which are infections anywhere along the urinary tract, including the bladder and kidneys, are the second most common type of infection in the United States. (1) Read on for five ways to prevent and treat a UTI without antibiotics.

UTIs can be caused by poor hygiene, impaired immune function, the overuse of antibiotics, the use of spermicides, and sexual intercourse. The most common cause, accounting for about 90 percent of all cases, is the transfer of Escherichia coli bacteria from the intestinal tract to the urinary tract.

For those of you who have experienced a UTI, there isn’t much you wouldn’t do to avoid another one. While I personally have never had a UTI, my patients have told me how symptoms like pain, burning, nausea, and even bloody urine can be debilitating, and for those who get chronic UTIs, the fear of infection can be enough to prevent engagement in any activities that could trigger one. For people who get them frequently, sometimes a specific cause cannot even be pinpointed. This can be frustrating and scary.

Fortunately, there are a few methods of natural UTI treatment and prevention that have worked extremely well for my patients, to the point where they no longer worry about getting a UTI.

These treatments don’t require a prescription, are inexpensive, and are completely drug-free. While your doctor may not know about them, I hope this article will help you completely avoid UTIs—or at least significantly reduce their frequency and severity. I recommend working with a Functional Medicine practitioner who is focused on treating the root cause of your UTIs.

What Is the Standard Treatment for a UTI?

Doctors typically use antibiotics to treat UTIs, and the type and duration depend on your health condition and the type of bacterium found in your urine. (2) Commonly prescribed antibiotics are:

  • Trimethoprim/sulfamethoxazole (Bactrim)
  • Amoxicillin
  • Ampicillin
  • Ciprofloxacin (Cipro)

These antibiotics are often unnecessary and may cause more problems in the future by destroying the beneficial bacteria that prevent pathogenic bacteria from growing. Long-term use of antibiotics can also lead to antibiotic resistance in strains of bacteria like E. coli in the gut, and a UTI caused by these bacteria will be even more challenging to eliminate and can cause more serious issues like a kidney or bladder infection.

Furthermore, antibiotics do very little to prevent the infection from happening in the first place. So, while drugs may be an easy fix for the short term, in the long run, you will continue to be susceptible to UTIs, and these infections may be worse than if you had never taken a course of antibiotics in the first place!

Curing and Preventing UTIs Naturally: D-Mannose

Treating UTIs with D-Mannose

D-mannose is by far the most effective supplement for both treatment and prevention of UTIs. Similar to glucose in structure, D-mannose is a naturally occurring sugar that is found in a number of fruits, including apples, blueberries, and cranberries. (3) This sugar is the reason that cranberry juice has been commonly recommended as a UTI treatment, though it is far easier to get the recommended dosage from a supplement. D-mannose is effective because it attaches to E. coli bacteria, causing them to stick to each other and preventing them from sticking to the walls of the urinary tract. (4) The bacteria can then easily be eliminated from the body during urination.

D-mannose, even in large quantities, does not cause any adverse effects, and cannot be metabolized the way other sugars can, meaning this supplement is safe for people with diabetes and others who are avoiding sugar for any reason. This treatment is also safe for children and the elderly.

Symptom relief can be seen as quickly as the following day, and most symptoms of UTI are generally resolved after 48 hours of treatment. Additionally, taking D-mannose during a time where you feel you are most prone to UTIs, such as before intercourse or during prolonged antibiotic treatment, can help prevent a UTI from ever developing in the first place. This is especially helpful for those who are prone to chronic UTIs and want to be able to engage in normal life activities without fear of infection.

The typical dose of D-mannose for UTI treatment is 500 mg, in capsule or powder form, taken with or in a glass of water or juice every two to three hours for five days. It is a good practice to continue taking the supplement even after symptoms have diminished to ensure complete elimination of the bacteria in the urinary tract. This dose can also be taken as a preventive, or prophylactic, method.

While there has not been any peer-reviewed research to support the effectiveness of D-mannose in treating or preventing UTIs, clinical and anecdotal experience suggests it is highly effective for the majority of infections, both acute and chronic. Some of my patients who have used D-mannose as a UTI treatment method have even described its effects as “miraculous”—so it’s definitely worth a shot!

Four Alternative Treatments for Chronic UTIs

One caveat with D-mannose is that it is only effective with UTIs caused by E. coli infection. While this accounts for about 90 percent of cases, 10 percent will not benefit from this treatment. In this case, supplements that help disrupt biofilms can be useful in treating and preventing UTIs.

Biofilms are an accumulation of microorganisms and their extracellular products forming structured communities attached to a surface such as the lining of the urogenital tract. (5) The development of a biofilm can make infections extremely hard to treat, since they commonly return shortly after treatment is stopped.

The antibacterial resistance of pathogenic biofilms is one of the major reasons why those who get a UTI are highly susceptible to getting more in the future—if the biofilm is not completely eliminated, the infection will eventually return at some point.

1. InterFase Plus and Biofilm Defense

This is why the use of biofilm disruptors can be helpful for preventing the recurrence of chronic UTIs. The biofilm disruptors that I recommend to my patients are InterFase Plus from Klaire Labs or Biofilm Defense from Kirkman. These contain specialized enzymes to disrupt the biofilm matrix embedding potential of pathogens, and dissolve the sugar and fibrin components of most pathogenic biofilms. By destroying the biofilms, the recurrence of UTIs despite proper hygiene can be reduced. (6)

2. Lauricidin

Lauricidin is another supplement that may help in treating UTIs, particularly those that are caused by types of bacteria other than E. coli. Lauricidin (a proprietary form of monolaurin) has antiviral, antifungal, and antibacterial activity, and is specific against pathogenic bacteria so it won’t disrupt beneficial bacteria in the gut. It is highly effective at combating gram-positive bacteria in the families of: (7)

  • Streptococcus
  • Staphylococcus
  • Corynebacterium
  • Listeria
  • Bacillus
  • Clostridium

It works by disturbing the integrity of the bacterial cell membrane, blocking replication and making it easier for the immune system to destroy the pathogen. Lauricidin is only helpful, however, for UTIs not caused by E. coli, which is gram-negative and has a different kind of outer cell membrane than gram-positive bacteria.

3. Nattokinase

Nattokinase is another enzyme that has been shown to dissolve biofilms. (8) Produced by the bacteria found in the fermented food natto, this enzyme is proteolytic and can help break down the fibrin proteins that maintain the structure of biofilms.

Because of its fibrin-breaking ability, it’s important that nattokinase supplements are not taken by people with bleeding disorders, or by people who are taking Coumadin (warfarin), aspirin, or any other drug that influences blood clotting, unless supervised by a physician.

4. Apolactoferrin

Apolactoferrin (or lactoferrin) is one more supplement that I recommend to my patients with recurrent UTIs. This multifunctional protein lactoferrin is a component of the immune system with antimicrobial activity, and is part of the innate defense, found mainly in secretions and mucosal surfaces. (9)

Lactoferrin has been shown to block pathogenic biofilm development by binding to iron and causing the bacteria to “wander” across surfaces instead of forming cell clusters and biofilms. (10, 11) One study found that the amount of E. coli bacteria in the kidneys and bladder of mice was significantly reduced 24 hours after oral lactoferrin treatment, compared to a control group. (12) More research is necessary to demonstrate the effectiveness of lactoferrin in treating UTIs, but I believe it is worth trying, especially if dealing with chronic UTIs.

      A combination of raw garlic, cayenne, and (15-20% oleuropein, 500 mg) olive leaf extract kills even antibiotic-resistant Klebsiella. (I’ve helped someone who’d had Klebsiella for 4 years)
      Take 1/4 tsp cayenne, 1 large clove minced raw garlic, and 4 olive leaf capsules per meal.
      You’ll want to take activated charcoal between meals (2 hours before and after eating) and start out with 1-2 olive leaf capsule and work your way up to 4.
      Most people have a lot of yeast in their gut after antibiotic use, and when those things kill that yeast it makes you feel sick, unless you alternate charcoal capsules with the other things.

    I’m writing on behalf of my girlfriend Sophie. She is in a desperate situation that is affecting just about every part of her life and I’ve been trying for years to find a way to help her as doctors and specialists have basically said they are out of ideas.
    Sophie suffers from Fowlers Syndrome, in a nut shell this means she struggles to pass urine leading to retention and of course UTI’s She self catheterises at least 4 times a day. Aside from the obvious trials of not being able to urinate properly she suffers with an infection maybe once a month. Although I feel these infections never actually go away they just vary in terms of symptoms. Each time she gets very poorly she is prescribed another dose of antibiotics that doesn’t work or clears it up only for the next infection to start. I read all bout D Mannose and as such she is now taking these but still can’t seem to be rid of infection or the varying symptoms. I’ve read so much and tried so many different things that I think I’m confused as to what to try next or what is the right course of action for her. We’ve been together 4 years and I feel that she is becoming more and more depressed with constantly feeling unwell and having light whatsoever at the end of the tunnel. I was instantly intrigued by this article as it names a number of supplements I have not heard of and explains how they work but I’m still confused as to where I should start with Sophie. Any advice would be much appreciated.

    • D-Mannose only helps with E. coli.

      I would suggest chiropractic treatment to make sure that the sacral nerve is functioning properly, valerian root to help the muscles relax, and a course of supplements that includes the use of olive leaf extract, raw garlic, ginger, turmeric, and cayenne pepper.

          If the nerve is impinged, a stimulator won’t do much good, because it’s like putting a brace on a paralyzed leg. I would look for a good chiropractor; it can’t hurt and it may help.

  2. Hi- I am hoping I can get some advice too. I’ve had a UTI about 2 weeks. It rapidly became an unbearable pain within the first 12 hours. I panicked and took bactrim and pyridium. It improved by the next morning. I took a 3 day course of bactrim and thought it was over. On the 5th day, I started with symptoms again. I panicked in fear of the pain, so I took 1 cipro. Then I panicked again and decided not to take anymore cipro when I found this posting. Since that day, I did 3 full days of cayanne 1/4 every 4 hours and baking soda water, plus olive leaf, grapeseed, and mega cranberry. I was overjoyed when symptoms began to disappear. It has been 3 days since stopping the regime, and I feel bladder fullness and urgency symptoms again with a strong smelling urine. Ugh!!!!! What do I do now? Should I start the regime all over again. I also bought the activated charcoal. Please help! I want to rid this in the most natural way possible!

    • You stopped too soon! You want to continue the full course for 2-3 days *after* a home UTI test is negative for infection, then you taper down to a maintenance dose, because once you’ve had a UTI, you’re at increased risk of re-infection over the next 6 months.
      So do what you did, and keep at it till you’ve completed at least 2 days past the day you tested free of infection. Then take one less capsule per meal than you were taking, and taper down each day till you’re taking one capsule of each supplement with breakfast and with dinner. Do that for 6 months as preventive maintenance, then drop to one olive leaf capsule with breakfast and dinner, like a daily vitamin. It’ll prevent pretty much everything, including the common cold.

        So should I continue with the cayenneor olive leaf? I started to take 4 olive leaf 2x per day and activated charcoal 2x per day, plus 1 tsp dmannose in morning and night. Would that be sufficient? What do you recommend as maintenance?

        • 4 olive leaf twice a day and 1/4 tsp cayenne 4x per day for 5 days should do the trick.
          Take a test the morning after the 5th day and see where you stand. You want to continue the full course for 2-3 days after you’ve tested clear of infection, then begin tapering down slowly to a maintenance level to prevent future infection.
          (2 olive leaf per day and 1 dose cayenne 3x per week.)

            • Do you think the d-mannose helped? It’s only effective for E. coli infections.

              I never tried cantharis; after reading the history of homeopathic medicine I concluded it was useless, but if you already have it, it’s worth a shot.
              Since what you did before with the cayenne and olive leaf worked, I would go back to that and hit it hard.

              • About the homeopathic remedy. I was raised on them, so I have no problem with them. The remedy simply works on the SYMPTOMS. It matches what is happening to the body. The D-mannose and cayenne pepper actually kill the bacteria. In my case, I got colloidal silver too which also kills bacteria. So they are different approaches to handling the condition. If your symptoms don’t match those of cantharis, then you need a different remedy. Many people have similar symptoms when they are infected with e.coli, that is why cantharis works for them. If you were to look up cantharis in a homeopathic book, it would describe the symptoms associated with UTIs.

    • Any brand should be fine, really, as long as they’re a reputable company that doesn’t adulterate. (Swanson’s is fine, if you see that)
      Activated charcoal is just superheated hardwood bark, or coconut hull; its benefits lie in its ability to draw things to itself.
      In this case, it draws up the toxins released when olive leaf rips yeast cells apart..

      So you don’t have to worry about strength or potency, since it’s an inert substance. The only thing that makes a difference is volume, the size of the capsule it’s in. Your average size capsule is going to be around 280mg; take two of those and see how your symptoms react. If you still have strong die-off symptoms, take another. Always remember to drink plenty of water with it, and don’t eat anything without 2 hours before or after taking. Just drink plenty of plain water.

      That olive leaf protocol is for emergency treatment of a UTI for someone in pain, but someone who has good cause to think they have a fair amount of yeast overgrowth needs to start gradually, with 1-2 olive leaf capsules per meal, and build up slowly to 4 per meal.

    I wondered if anyone is able to help me, I care for my mother who has dementia, she has been suffering from UTI’s for years all has been treated with antibiotics last year mum has a very bad uti which turned into urosepsis, so where as the previous years she has been free for 1 month before the infection reappears again. this has decreased to 3 weeks. The infection makes her very agitated shouting day and night which means she doesn’t get much sleep for the 8 days, wondered if anyone had an suggestions that I could try. as she has the bacteria in her urine called Citrobacter Freundii. I use the dip sticks to test her urine regularly.

    • Wow, I’m so sorry!

      It sounds like she’s likely to have a lot of yeast, which will have to be dealt with slowly, but you could try 1/4 tsp cayenne pepper in a spoonful of honey. When it works, there’s significant pain relief within 90 minutes.

  5. Ok — I have FINALLY FOUND PEACE after a string of UTI’s and over a year without being able to have sex with my new partner. Here is my story:

    I met someone new and started having sex. This catapulted me into a year and a half of chronic and recurring UTIs that no one could find a lasting solution for or explain what was happening…

    I took 3 rounds of antibiotics (cipro, macrobid, cipro) for 3 e.coli infections which left my body in bad shape. The next UTI I got I treated herbally with teas.
    Lactoferrin, BioPhase, Nattokinase might sound good from a biomedical theory perspective, but did absolutely nothing for my chronic and recurring infections. Lactoferrin completely robbed my body of iron; my nails began crumbling and I stopped menstruating after a month on it and my infection persisted. I think that once antibiotics have annihilated the immune system’s ability to function properly, these supplements don’t address the underlying issue which is: 1) toxic buildup in the major detoxifying organs (kidney, bladder, liver) caused by overactive pathogenic microbes (bad gut flora) and 2) an overwhelmed, exhausted and under-functioning or non-functioning immune system.

    What would amazingly cure my first 3 post antibiotic infections but not prevent recurrence (as I was continuing to have sex with my partner when I felt symptom free):
    I drank a 2 quarts/day of tea recommended by a local herbalist consisting of Buchu leaves (antimicrobial), Couchgrass (soothing), Uva Ursi (urinary antimicrobial), and Ecchinacea Augustafolia ROOT (immune boosting-purpurea apparently is not as immune stimulating). Ecchinacea root simmered gently and COVERED for 25 mins, Couchgrass for 10 minutes; Uva Ursi simmered gently for 4-7 minuts, turn off the stove and add Buchu Leaves for 3 minutes covered then strain the whole concoction into a jar. Drink within 24 hours!
    -Western red cedar tincture–2 dropperfuls, 3 times per day.
    -Juniper berries: ground them in the blender and steeped (1/2 cup) for 15 minutes in 4 cups of water to be drunk throughout the day.

    There is some writing that warns of continued use of Buchu, Uva Ursi, and Juniper berries for periods of more than 2 weeks and my experience confirms this. Please use these cautiously and if they are not working, try a different tea formula with gentler herbs like pipsissewa (or blueberry leaf), goldenseal (not gentle, but very antimicrobial), yarrow, licorice.

    I cut out all sugars, almost all carbs, from my diet and stuck with an autoimmune paleo diet. No honey, maple syrup, white potatoes, grains, carrots, etc. I do very limited but regular sweet potatoes and unsweetened canned pumpkin (yum!) for carbs because I realized that going no-carb was starving my good bacteria thus preventing them from flourishing and repopulating my urethra/intestines/whole body! Probiotics needs healthy starches to survive! Ultra low carb diets = starving healthy bacteria = starving immune system! EAT SMALL AMOUNTS OF RESISTANT STARCH REGULARLY, EVERY DAY!

    But, even with an amazing diet and amazing supplements, I continued to not be able to have sex without getting infected and I also still felt like absolute hell mentally, digestively, physically, etc etc etc. This was extremely depressing. The struggle went on and on until….

    REAL RELIEF:

    About 6 months ago, I started going to a Traditional Chinese Medicine Practitioner–specifically, a “5 elements” TCM practitioner. 5 Elements goes a little deeper and is more all-encompassing. This TCM practitioner also knew homeopathy which ended up being extremely, extremely important.

    When I first went to see him, I had started taking an anti-parasite/anti-fungal herbal formula that my partner and I found online (called Humaworm) as an experiment after reading about all the havoc parasites can wreak on the immune system (seemed to explain my condition perfectly). Through all the insane treatments I had undergone, none of them had addressed the parasite/yeast/microbial overgrowth component. At this point I was still dealing with an active infection and mentioned to him that I suspected parasites. After a week on the formula I was feeling GREAT (better than I had in years) and my UTI symptoms were down to almost nothing. The TCM doctor agreed that I was onto something and treated me with acupuncture alone in hopes that that would be all I needed to clear my UTI and he wanted to gauge the severerity of what he called my Gu Syndrome. We tried this for about 3 weeks, and I had very minimal UTI symptoms this whole time and I continued with the Humaworm. But sadly, once I finished the anti-parasite/anti-fungal formula (Humaworm), I immediately had a nasty flare up (all of my UTI symptoms returned) so in my next appointment with him once he saw how my symptoms had returned (I was devestated), he put me on 7 different homeopathic remedies and an chinese herbal formula for clearing damp heat called Fu Ling Poria.

    Each homeopathic I was put on serves a different function, but here is my general understanding of what their role is: the “Unda” formulas I am on operate on a very deep level to clear out the toxic debris from the years of fungal/bacterial/parasite waste and other pollutants. They also support the kidney and improve it’s function which supports the bladder, etc. Then, other homeopathics I am taking stimulate or “reboot” the immune system while another homeopathic formula irritates or “wakes up” the pathogens that are hiding in biofilms so they will come out and be eradicated by the now-functioning immune cells.

    Here are the homeopathic formulas he put me on first:
    Unda 2
    Unda 7
    Unda 44
    Syntrion – Syinfect Complex
    Staphysagria 9CH
    Cantharis 30k
    Medorrhinum 15c

    My urinary infection symptoms improved dramatically when I started these formulas and after about 2 weeks my infection cleared up completely. Since this initial round of homeopathics I have been on a variety of different ones, helping to soothe my digestion and repair the havoc wreaked by parasites, yeasts, etc. I have not had another UTI.

    His diagnoses: I have Gu Syndrome. This is basically what Chinese Medicine calls a major parasite/yeast/microbial infection. This explains why things get so much worse after anti-biotics. Antibiotics allow Gu to take over because so many of the good bacteria are annihilated. Due to an onslaught of infection and the toxins produced by all these organisms, my immune system was basically exhausted and my digestive system is chronically irritated and not functioning properly (Leaky Gut). Due to the constant stress of a variety of fungal, bacterial and parasitic infections (GU Syndrome) and then an onslaught of antibiotics, my immune system stopped functioning because the battle was too big. My immune system could no longer mobilize aggressively enough to take care of invaders like E. Coli before it is too late. It also doesn’t mobilize to keep me free of additional fungal/bacterial/parasite overgrowths — vicious cycle!

    The trick is that Gu Syndrome takes time to heal from because you have to strengthen the body’s immune system without strengthening the organisms that are doing all the damage (compounds like Ginseng can make Gu Syndrome more ferocious, for example). So, I see my TCM doctor twice a month and he puts me on a carefully selected batch of chinese herbs that combat the Gu while gently supporting all the other symptoms of my body.

    6 months later I have continue to feel more and more healthy and CAN HAVE SEX WITHOUT GETTING AN INFECTION.

    I am still on an auto-immune paleo diet. I am able to have sex and fool around regularly after a year of not being able to have any sexual contact whatsoever. My partner and I use female condoms — if you are prone to UTIs, USE FEMALE CONDOMS (FC2). THEY ARE AMAZING. Not only do they feel great but they protect your urethra and help keep the junk out. Also make sure your partner (if they have boy anatomy) takes a shower after they poop(seriously). My partner and I have dabbled in condom-free sex and I have not gotten a UTI (yay!) but we are still playing it safe until my body is back to full health. I see my TCM doctor twice/month and am on homeopathics and herbs every day.

    In my opinion, without addressing the underlying strain on the immune system, the body will continue to struggle with UTIs. After paying thousands of dollars on naturopaths, supplements, blood work, urinary specialists, herbalists — 5 Element Traditional Chinese Medicine and Homeopathy are what finally solved the puzzle.

    Thanks so much for your useful comments and especially the flowchart you’ve provided. I have had issues with infections/something else (last culture was negative – but from what I can gather this doesn’t necessarily mean no infection, and culture prior to that was positive) and have been on trimethoprim, nitrofurantoin (when culture showed infection was trimethoprim resistant) and then co-amoxiclav when the infection symptoms returned 10 or so days after finishing the nitrofurantoin. I’ve been good for about 14 days minus a few tell-tale signs, but I can feel things are about to take a turn for the worse. I’ve been using dipsticks throughout which have been negative for all three indicators. This morning my leucocytes are a little raised, but this result is only showing about 5 minutes after the dip, which means it’s diagnostically null – but I do wonder if it means positive in some way.

    I have no particular questions at the moment and just want to say thank you, as I was looking for some form of solid natural regime as I do not want to go on antibiotics again. I had a mild yeast infection some days after finishing the co-amoxiclav, which is a miracle as I normally get yeast infections about two days into any course of antibiotics. I’m going to try the cayenne later.

    • I would strongly suspect an intestinal yeast overgrowth in this case, and just so you know, a lot of yeast can deflect cayenne pepper so that it never reaches your bladder.

      If you decide to do the olive leaf/charcoal cleanse treatment, build up gradually with the olive leaf, starting out with 1 or 2 per meal the first day, and slowly increase.

      • I’ve already got some activated charcoal so have just started that. Will be buying the olive leaf extract from Amazon tomorrow. Do you recommend doses higher than 500mg? Some of the ones on there seem to be around 600-700mg.

        I’ve taken a couple of cayenne doses today and I can’t really tell if it’s made a difference. My symptoms are quite vague to begin with, so that doesn’t help. Will report back with further updates! I’m devastated this seems to be happening again. I very much refused to believe that was going to be it after the last antibiotics and had only just started letting myself even entertain the thought I might have just had a difficult infection but that it had gone.

        • Ohhhh…I know that feeling!
          You try to convince yourself you’re imagining it, because you just Do Not Want this crap! Not again!

          Olive leaf is the bomb. I mean it, it’s a freakin’ bomb when you have yeast. Believe me, 500 mg is sufficient.
          You should probably start off taking one with each meal, just in case. I generally recommend two, but that’s too strong a start for some people. Follow the timing in the chart linked in my screen name in some of these comments, but start taking one per meal the first day, and two per meal the second day, and see how you feel. You may need to do 2 days of 2 per meal before you up it to 3.
          4 per meal without having to take charcoal is the goal for a complete kill of yeast and UTI, but you have to work your way up to it when yeast is involved, even though you’re taking charcoal. Make sure not to eat anything within 2 hours before or after taking charcoal, and drink lots of water in that time!

          • I’ve ended up buying 750 mg capsules, based on a mix of needing veggie caps, positive reviews, price and needing this to be delivered ASAP (so, Amazon Prime is my best bet). I’ll maybe try to split the doses initially in case I can’t tolerate it.

            What are your thoughts on apple cider vinegar? My plan is to follow your regimen of cayenne (which doesn’t seem to be affecting my adversely at least, so far), charcoal and OLE, alongside a few cloves of raw garlic a day, but I’m thinking I might as well try ACV too. I don’t want to take too much, but I definitely want to try to tackle this before it gets out of hand and I’m in a crisis scenario needing antibiotics again!

            • ACV is fine for temporary pain relief, but for something to take for your pH when you’re taking things like cayenne and garlic, I suggest a glass of baking soda water, because it softens the effect of those strong medicines on your digestive tract without reducing their effectiveness.
              Be careful with that olive leaf! Start slow. If you’ve got a lot of yeast, slamming it with olive leaf is like being kicked by a horse.
              Be careful to take your charcoal; you can take one capsule as soon as you feel sick, and take the second at the mid-way point between mealtimes.

              • Hi Beckah, what do you mean by take the charcoal when I feel sick? Should I be feeling sick? lol

                A small update: I’m doing the cayenne and the charcoal and will be starting the OLE today. My symptoms are still at that point of ‘something doesn’t feel right’ but aren’t full blown. It’s a feeling I actually had for six months on and off (treated just by upping fluid intake) following a UTI last year (prior to that, I had only had 1-2 uncomplicated UTIs in my lifetime). I drank some bicarb water before bed because during the night is when it tends to strike for me, and I managed to not get up during the night.

                I’ve hold hold of my lab results from Jan and Feb. The Feb result was clear (‘No significant growth 10^5 cfu/ml) and was resistant to amoxicillin and trimethoprim, and sensitive to cephradine, nitrofurintoin (tried) and co-amoxiclav (tried). I’ll press on with the treatment plan and might give D-mannose a go if it doesn’t work.

                • That olive leaf will kill yeast in your intestines before it does anything else, and when yeast dies it spills out toxins. Those give you headaches, nausea, diarrhea, and just generally make you feel horrible.
                  What charcoal does is it adsorbs, draws those toxins into itself, and carries them out of you so that their ill effects are minimized. It’s like a vacuum.
                  In order for it to work properly, it has to be alone in your system with plenty of plain water. So, you take the olive leaf, and chase it with charcoal. The charcoal vacuums up what the olive leaf destroys, and you’re rid of your yeast quickly. Yeast acts like a defensive line for a UTI; it runs interference for it against the things you take.
                  Antibiotics pass right through it because yeast can’t eat it and it doesn’t kill yeast, so they basically ignore each other. Other things, like D-Mannose, never get past it because sugar is yeast food. Some things kill just enough yeast to make you feel sick, and you think that what you’re taking is making you worse, and not helping your UTI, when it *would* help you if it could get past the yeast and into your bloodstream.

                  When olive leaf kills off the yeast in your gut, it moves into your bloodstream and ends up in your bladder, where it destroys UTI pathogens. For most women, this takes a few days, but from there on out improvement is rapid.

      • Is it normal that I’m not having signs of sickness or being unwell at all with the OLE? I’m taking 1 x 750 mg pill three times a day. I’m also doing the cayenne and charcoal, but still so far only taking 1 charcoal pill (I forgot I was supposed to take 2). On the positive side, I’m not feeling that feeling of impending UTI quite so much. I am feeling something still, though, which is very hard to explain — not a pain, not a discomfort, but more like a sensation. But no frequency, urgency or burning/stinging. I’m starting to suspect there’s something going on other than UTI — IC, maybe.

        • That means you don’t have a yeast overgrowth, which is good!
          Olive leaf doesn’t make you feel bad; dying yeast makes you feel bad. If you can take a large dose of quality olive leaf and feel fine, that just means your gut is healthy.

          • So does that mean OLE doesn’t actually help UTI unless there’s yeast involvement (which apparently there may not be in my case)? I’ll keep on taking it anyway, but I’m curious as to if it can work in that ay.

            • Oh, OLE is effective against UTI pathogens alright, it’s just that it kills yeast too, and, when you have it, intestinal yeast is the first thing it runs into.
              So OLE *is* killing your UTI right now, and it didn’t have to fight its way through yeast to get there.
              That’s great!

      I gave birth 3 weeks ago and now my urine has a fool and strong smell to it. I have lower back and groin pain, but I contribute that to the birthing, since I had those prior as well.
      Are the above mentioned remedies safe now that I am lactating?

      • Yes! I took several of them while nursing my last baby; the only thing I would have any concern about is the cayenne. That, I didn’t take while the baby was nursing just because I hadn’t discovered it yet.
        I actually have a lady who’s nursing a young baby and taking a dose or two of cayenne a day with no effects to the baby, so go ahead and try it starting with a 1/8 tsp dose in a spoonful of honey.
        Wait and see how you feel (you may experience a warmth/tingling in your bladder, which means it’s working) and how the baby does, and if you feel like your symptoms improved and the baby doesn’t exhibit discomfort or loose(r) stools, take a little more than that 4 hours later. I think you’re safe to take 2 full 1/4 doses a day (with food) without the baby even noticing. Chase each full dose with a glass of baking soda water, 1/2 tsp dissolved in 8 oz water.

        Olive leaf is totally safe, charcoal is an inert substance that doesn’t cross into your bloodstream so it has no impact on your milk at all, and I took large amounts of raw garlic without the baby ever reacting (other than that it made her cold better, when she got one:), so you can be totally comfortable with these things. 🙂

  7. I have a uti at the moment from a bacteria I haven’t seen mentioned in the comments. It’s Morganella Morganii. I have no idea how I got this particular bacteria but I’m in my third round of antibiotics to get rid of it. Any suggestions?

  8. This is most interesting information and I’m so happy to see some recommendations other than antibiotics followed by more antibiotics when the first ones don’t work. I am curious, though, about how cayenne and/or D-mannose and/or olive leaf will help people with MS who have recurring UTI’s. MS-related UTI’s don’t necessarily seem to be related to infection, nor do I necessarily have the same symptoms (burning, urgency, etc). Often the symptoms mimic an MS-exacerbation (rise in temperature, confusion, , extraordinary fatigue, lower abdominal discomfort) and it’s difficult to determine whether it’s UTI or MS. I’m thinking nonetheless that the cayenne/honey treatment is a good first-line defense. Any comments/suggestions are appreciated. 🙂

    • I would try the cayenne first and go on a maintenance regimen of one (strong) olive leaf capsule with breakfast and dinner, and get a big bottle of UTI test strips so you can check on things whenever you want.

      • I have never heard of UTI test strips. I have some PH strips though. Today is my last day of cayenne and my olive leaf and charcoal will be arriving tomorrow afternoon. I have never had any normal symptoms for UTIs like burning, ect. so won’t know when mine leaves. So do these UTI test strips really tell when you are free of it? Are they easy to get at a local pharmacy?

        • Oh yes, they test your leukocyte (white blood cells) count, which indicates infection and inflammation, and test for nitrites, which indicates the presence of gram-negative bacteria like E. coli, Klebsiella, etc..

      • Thank you, Bekah, for your suggestions. I took the cayenne tx twice the first two days and have noticed an improvement already. I missed my dose today, so we’ll see if the symptoms recur.         • 1/4 tsp 4x a day for 3-5 days! Then one dose per meal for two more days, then twice a day for two days, then once a day for prevention, along with a capsule of olive leaf extract.
          Take regular preventive maintenance doses very seriously to keep this from recurring; these things really do try to haunt you.

      • Thank you again, Bekah, for your most useful suggestions. I took two doses of cayenne in honey for two days, then one a day for about four days. The heaviness in my lower gut subsided, the horrific smell is gone, and I just know it helped. However, after the final dose the day before yesterday, I developed pain in my upper right abdomen and across my upper belly and also expelled lots of gas. Generally, I felt dreadful all day. I took my first OLE with dinner last night (I had trouble getting it), then charcoal with water later in the evening. Today it’s somewhat better, but the pain is still noticeable on palpation. I’m thinking I ought to continue with the OLE/charcoal cleanse for a few days, ‘cos I feel there are loads of toxins in my system. So, when I get up in the a.m., do I take the charcoal first and wait a couple of hours to eat and take the OLE, or do I eat first, then wait a couple of hours until after eating and taking the OLE?

        • By the way, it’s almost impossible to buy UTI home testing strips in Canada. The Easy Home strips you recommend via Amazon aren’t shipped to us here. I did find one drugstore locally that sells three test strips for $12.99, which will get rather pricey if I’m going to test for a week or so! Still searching for a better and easier to obtain bargain …

          • Well, as long as your symptoms are subsiding, you have evidence that the infection is being defeated, so you could get one box and wait until you’ve had 0 symptoms of your infection for 2 full days, then test.
            If it’s totally clear, do the full protocol for 2 more days and test again, then start gradually tapering down and test again in 5 days, when you’re at a preventive maintenance level.

        • That’s the yeast dying! The olive leaf will speed that up a lot and help the cayenne kill your UTI, as well.
          You can take charcoal both before and after your supplements as long as you keep that 2 hour buffer in there where all you do is drink water.

        • Oh, and KEEP taking the cayenne! You can take it twice a day if you’re taking olive leaf extract with every meal.

            • Yes, you need to keep taking things for days after your symptoms are gone! You don’t want it getting up and limping back to harass you anymore; kill it with extreme prejudice.

  9. My 4 year old daughter has has bladder issues since last summer. Lots of accidents, bed wetting and this January I noticed some dark brown/red discharge. We’ve had multiple urine cultures done, 2 ecoli positive and the one In between was clean…after flushing her out with a lot of water, AZO tablets, using essential oils and nano colloidal silver on/off. So she’s positive with ecoli again-bed wetting nightly and can’t keep her underwear dry!! My alternative Doctor recommended “bladrease” an herbal blend with fennel, bilberry, witch hazel, and burdock which she’s been and seemed to help, but now it’s all coming back. Now they want her on antibiotics of course. Just bought d-mannose and started yesterday but not sure how much to give her since she’s so young. She’s only had 1 capsule (500 mg) daily the last 2 days. I found your website this morning and just gave her 1/4 tsp cayenne pepper (put it in veggie caps). I’m hoping that wasn’t too much for her!! I would love any advice on what to do with her and what dosage for all the supplements since she’s only 4. I have olive leaf extract and charcoal but I’m not sure if I should stick with the d-mannose and cayenne at first. And FYI I am very against antibiotic use–her last dose was dec 2014. Thanks in advance!!

    • I would give her 1/8 tsp cayenne pepper mixed into a spoonful of honey twice a day, once in the morning and once before bed, and give her food a light sprinkling of olive leaf extract powder every time she eats.
      The charcoal is only necessary if she has yeast die-off symptoms, and then I would swirl the contents of a charcoal capsule in a 4 oz water bottle and have her drink it with a straw. It’s a little gritty, but tasteless. Give her a few sips and cap the bottle tightly in between uses. Throw out anything left at the end of the day; charcoal water adsorbs things out of the air.

      • Okay thanks! How long should I keep her on the cayenne regimen? And the oliveleaf extract? Also how much d-mannose should I be giving her daily and then preventatively when she feels better?

        • Get some home UTI test strips (I’ve linked to a great deal on them on Amazon) and when she’s tested completely clear of infection for 3 days straight, go to a maintenance schedule of one dose of cayenne and one olive leaf capsule sprinkled on her food, per day. This will also protect her from colds.

          • OK thank you! We are in a maintenance phase now with her– how long does that last? Weeks, months? Since she’s tested for E. coli I’ve been giving her d-mannose as well as the cayenne and olive leaf extract Is that safe for young girl? How much d-mannose is good for her (4yrs) for prophylaxis? Also, can Cayenne hurt her little tummy if she takes it long term?

            • I’m so glad you got her infection cleared up that quickly!
              The best thing for maintenance is the olive leaf, because that protects against all forms of infection; the d-mannose only helps against E. coli.
              Is this her first infection? This comment reply form is way at the bottom of the 1,000 comment thread, so I can’t see your earlier comment until this one posts. If she doesn’t have a prior history of infection, you could do one olive leaf capsule a day for 6 months, then 3x a week as permanent preventive maintenance.

  10. Hello I have a question about a uti I am treating, or at least I think I’m still treating it. After reading the comments I tried a few of the suggestions.Last week on Tuesday on Wednesday I was having the burning pain so I tried the cayanne pepper but I had no luck. Maybe I was doing it wrong? On Saturday I picked up some d-mannose powder and was taking that every 2-3 hours for the rest of the weekend. The burning pain went away (that stuff is amazing)A day or two after taking it. This led me to believe the uti was E. coli related, I’m still taking the d-mannose once in the morning and the every few hours after I get home from work. Today, almost a week after the burning went away, I’ve noticed this mild lower back pain on my left side. I do sit at a desk all day at work. I would also like to add that My Urine is this weird very very light/clear greenish color. Sometimes it’s the normal yellowy color other times it’s Green. Is this uti related and Should I be worried? Shall I continue to take the d-mannose? If so how long for? Are there any other remedies to treat this besides antibiotics? I had a uti a few weeks ago and I would rather not take antibiotics again.

    • Do you take a supplement high in B vitamins? That can give urine a green tint. You may just be noticing it now because you’re paying particular attention.

      When cayenne doesn’t work on an infection, olive leaf extract does, especially combined with something else, like raw garlic or grapefruit seed extract.
      The thing to remember is that you can experience yeast die-off from olive leaf, which is best mitigated with activated charcoal.
      I made some comments on that, and how to alternate them to maximize their effectiveness, a little way down the page.

  11. Hi there! I read most of the comments and now I need some action on my part. I had a diagnosis of a UTI and was given some antibiotics. The following lab test said I still had a UTI so was given another antibiotic, which didn’t work either. Then the lab called and said the doctor had prescribed another antibiotic and they wanted to know where I wanted to pick it up. I told them and immediately called the pharmacy and told them to destroy that prescription because I didn’t want it. I was already worried about my “good bugs” disappearing in my gut. I finally found out that I didn’t have e-coli and was a bacteria of some sort. So now I am wondering just what I ought to be taking to get rid of it and then do a maintenance. If I read right I think it should be olive leaf extract capsules with activated charcoal tablets. And if that is right, what amount should I use? It seems that there are several things to take so I want to make sure I take the right supplements. Of course I won’t tell the doctor I got rid of the prescription because that would make her mad.

    • The first thing to try should always be cayenne pepper, because when it works on a UTI, it works like magic.
      As in, 90% of your symptoms gone in 2 doses of 1/4 teaspoon.

      However, with your antibiotics history, you want olive leaf and charcoal anyway because yeast will be going wild in there!
      You need 15-20% oleuropein, 500 mg olive leaf; Vitacost is an excellent brand. It’s on Amazon, and they have their own vitacost.com site.
      You alternate the two, strictly keeping them two hours apart, and take olive leaf with meals, and charcoal with water ONLY.
      Start with 2 olive leaf capsules each time the first day, and build up according to how well you feel. Olive leaf kills yeast and yeast makes you feel horrible when it dies, but that’s what the charcoal is for. It soaks up the released toxins so they aren’t absorbed into your bloodstream. You’ll probably still feel a little ‘off’, and your bowels will reflect that there’s something going on, but if you feel okay the next day you can take 3 with breakfast and lunch and see how that feels.

      Build up to 4 per meal and start dropping the charcoal; the yeast should be largely gone by then and most of that olive leaf will be getting busy with the UTI, if cayenne didn’t kill it.

      When you can take 4 olive leaf at a time and feel fine, with normal bowels, you’re clear of yeast. After that, take one with breakfast and one with dinner as preventive maintenance against just about everything.

      • Thanks so much, Bekah. I would prefer taking the cayenne in a capsule so how many mgs would that be? I see that varies and want to get the right one. Also how any mgs should be charcoal be? I want to order them today. One of my problems is that I don’t have the normal burning sensations with my UTI. I was not even aware that I had the problem until they did a urinalisis because I was so tired.

        • I don’t think it matters too much on the cayenne, because the cayenne I took, that cured me of an infection I’d had for YEARS, was from an old, expired bottle I’d had since before the UTI, itself!
          If you have a bottle in your spice rack, you can mix 1/4 tsp into a spoonful of honey and see if it works for you, before you buy a bottle of capsules. You’ll know within an hour or so, because it tingles and your symptoms begin improving rapidly.
          I think just about any kind you get will work, since expired stuff in an old bottle worked for me.

          On the charcoal, that’s an inert substance so it doesn’t have a ‘strength’, per se. You also don’t need much, because it’s so effective you don’t generally have to take it for very long.
          I would say get a bottle of it from a brand you trust, and take 2 capsules each time.

  12. My advice to everyone here would be to buy some urine test strips. Here in Australia they’re called Combur 10 Test (not cheap but it contains 100 test strips) and when you test your urine you can see when you have leukocytes or nitrites which a very common in UTIs and you can check the amount of blood in your urine. So you’ll know whether you have a UTI or cystitis, although doctors don’t believe in cystitis very much and my doctor told me that Cystitis is a bugger to figure out and treat.

    • Also meant to say that if you have these test strips you won’t need to go to the doctor first to test your urine which is unbearable anyway waiting for an appointment while you have pain and burning, let alone getting enough wee out when you’re just dribbling, not enough for a test at the surgery on the spot (at least when you’re at home you can time your wee to get enough in a bottle – make sure it’s midstream)… and if you have cystitis you can treat yourself and save yourself the cost of a doctor’s appointment. If you do go to the doctor, take the bottle+wee. Hence the bottle and test strips to do at home.

    • 1 cayenne capsule and 1 (15-20% oleuropein, 500 mg) olive leaf extract capsule per day would be ideal, for complete UTI prevention.

I got my first UTI when I was 17, and that was 31 years ago, I think I have had at least 30 UTI’s since then, and countless rounds of antibiotics, and that first round of Cipro was so brutal, it worked but just wiped out my gastro-intestinal system, it is a horrible feeling not being able to digest food, and all the gas and bloating. Your article is so amazing describing the difference between E-coli and non E-coli UTI’s. I have been fighting yet another UTI for two months now. I had not had one in a couple of years, and found myself in an ER and did a round of macro-bid. It did the trick, but it lowered my immune system and I caught the worst cold I have had in years, and UTI symptoms have come back 4 times in a month and a half. I went to the hospital to get my medical record, and lo and behold it is Klebsiella oxytoca, and because I read your article I looked up online and found out that this bacteria is gram negative, which explains its recurrence. I ordered Interfase Plus, Biofilm Defense, and Lauricidin online yesterday, and I eagerly await to try these supplements. Thank you so much, Chris, for writing this article. I have been suffering for decades with this issue!

  14. Hello. Ive had UTI’s on and off now since high school (now 26). I’ve had multiple cases each year…to the point where my gynecologist prescribed me antibiotics to take once a day when I was having sexual intercourse (which I now realize is a BIG no-no).

    I’ve been UTI free for months now and then the past 2 weeks have had one. I am refusing to go to the DR and trying home remedies. The AZO pills are life savers at the moment. I saw your article and got all the items I need . I started with cayenne a few days ago. and then for the past 2 days have been doing the grapefruit seed powder and olive leaf. oh and D-mannose too. So far, nothing. At night I feel good, and then in the AM I get the pain again.

    I have to go on a cruise this week and would hate for this to be in the back of my mind. I KNOW i probably have a yeast overgrowth from the excessive amount of antibiotics I’ve taken in the past. Should I start with the charcoal treatment? I am so prone to UTI’s I’m wondering what I can do to get this covered and done with (at least the pain part) in a week. Any advice is helpful.

    • Yes, when you don’t get a response to treatment that means yeast is in the way, in my experience.
      I’m betting that your olive leaf isn’t potent enough; what is the % of oleuropein, and what size are the capsules?

      15-20% oleuropein, 500 mg is what you want. If you’re not getting any response to your olive leaf, it’s not a quality brand or not concentrated enough.

      Good olive leaf, without charcoal, will have you running to the bathroom when you have yeast.

  15. I’m really happy to have found this informative article. I had a mild UTI and took D-Mannose. The symptoms cleared up in a few days. However, the same sensations have returned. I think I’ve had a low grade UTI for a while and I didn’t realize it and just ignored it until the symptoms got worse, i.e. more frequent urination with burning. I started on the D-Mannose again and have ordered Interfase Plus by Klaire Labs.

    My question is 1) I’m assuming I should take the D-Mannose and Interfase Plus at the same time? and 2) once symptoms are gone is it safe to stop the supplements or do I need to continue for a period of time to insure that this problem is fixed? Thanks.

    • Go in the kitchen and take 1/4 tsp of cayenne pepper. You can mix it into a little honey. If it’s effective against your infection, it will begin working within an hour and you’ll feel a warmth in your bladder followed by a sharp decline in symptoms.
      Repeat dose every 4 waking hours for a total of 4 doses a day.
      In three days, you’re cured.

      • Would cayenne pepper have interference with someone’s high blood pressure, diabetic, cholesterol medicines?

        • Cayenne naturally lowers blood pressure and LDL cholesterol levels. It thins the blood and increases circulation.

          If you try one dose and feel it working for you (1 dose is all it takes to know if it’s effective), then you could take the 4 doses per day of 1/4 tsp for 2 days and take a UTI test.
          Monitor your blood pressure carefully, but if cayenne does the job for you..you might consider using it for that instead of the prescription meds. It’s a lot cheaper, with fewer side effects, and helps protect you from future UTI’s.
          Something to think about, and discuss with your doctor and a naturopath.

    I just experienced what everyone here has. Some people like to hear details so here they are. I am male, 72 yrs old in relatively good health. This is my first UTI.
    It started Friday when I noticed the burning and I thought I haven’t been drinking water so I started. By Saturday it had gotten worse, so I remembered my old information and got some cranberry concentrate and parsley and added those to the water. The condition didn’t improve and Sunday I was going to the bathroom every 20 minutes. I found this website and took notice of the recommended supplements.     I am convinced natural products do work, if they don’t, it could be a biofilm problem. That is what I am thinking I have. I do not have a e-coli infection but a gram positive one. It has been reoccurring after I stop antibiotics without fail. I learned that these nasty bugs actually protect themselves from antibiotics and natural products by forming a jelly like substance around them. I liken it to a egg, the yolk is the infection and the egg white is what protects it. The antibiotics or natural products may not work if it cannot reach the infection. Biofilms can be very difficult to break down. I have read that NAC is great in helping the biofilms to break down. I am adding this to other biofilm disruptors I am taking. So maybe if we continue on the natural course of healing we can learn more about how to break down biofilms for those of us who get infection after infection. I have tried so many natural things, from oregano oil, to essential oils, coconut oil, cinnamon….the list goes on …but I really believe I need to get rid of the protection that the biofilms make before they will really work. Please any thoughts?

    • In my experience, a UTI that does not respond to strong medicine is being protected by a yeast colony in the gut.
      Women who’ve had no response to medication, natural or otherwise, and had no response to biofilm disruptors either, began responding normally to things like olive leaf and raw garlic as soon as they’d cleansed their gut with activated charcoal.
      Everything they took before then was absorbed by the yeast and never made it into their bloodstream in sufficient quantities to affect their UTI.
